docs: document provider signing and prepare 0.4.0
README gains a provider table, the sign/verify snippet, the CLI examples
and the note that the Feishu bot signature does not cover the message
body; SCOPE.md lists provider signing as in scope and keeps event payload
parsing explicitly out of scope; CHANGELOG records 0.4.0.
